Moderna Cancer Vaccine Shows Promising Late-Stage Results

Moderna and Merck have reported encouraging results from a late-stage clinical trial of a personalised mRNA cancer vaccine designed to reduce the risk of melanoma returning or spreading after surgery. The vaccine was tested alongside Merck’s immunotherapy drug Keytruda, also known as pembrolizumab.

According to the companies, the combination achieved the trial’s primary goal of reducing the risk of cancer recurrence. It also met a secondary goal by lowering the risk of melanoma spreading to distant parts of the body.

The results represent a significant milestone for personalised cancer vaccines, an emerging approach that aims to use a patient’s own tumour information to help the immune system recognise and attack cancer cells.

How the Personalised mRNA Cancer Vaccine Works

Unlike conventional vaccines that prevent infectious diseases, personalised cancer vaccines are designed specifically for individual patients.

After a tumour is removed through surgery, researchers can analyse its genetic mutations and identify features that could help the immune system distinguish cancer cells from healthy cells. Moderna’s mRNA technology then provides instructions designed to stimulate an immune response against those tumour-specific targets.

The aim is to help the immune system identify and destroy remaining cancer cells before they have an opportunity to grow or spread.

This approach forms part of a broader shift towards precision oncology, where treatments are increasingly tailored to the biological characteristics of an individual patient’s cancer.

Moderna Cancer Vaccine

Phase 3 Trial Included More Than 1,100 Patients

The Phase 3 trial involved 1,137 patients with high-risk stage IIB to IV melanoma whose tumours had been surgically removed.

Participants were divided into two groups. One received Keytruda combined with Moderna’s personalised mRNA vaccine, while the other received Keytruda alone for approximately one year.

The companies said the combination reduced the risk of melanoma returning and also reduced the risk of distant metastasis.

Importantly, Moderna and Merck said no new safety signals emerged during the trial.

However, detailed findings have not yet been released. The companies are expected to present more comprehensive results at an upcoming medical meeting. Overall-survival data, which could show whether the treatment helps patients live longer, are also still pending.

Why the Results Matter

The findings are particularly significant because Keytruda is already an established treatment for melanoma. If adding a personalised mRNA vaccine can improve outcomes for patients receiving the immunotherapy, it could strengthen the case for combining cancer vaccines with existing treatments.

The companies described the findings as the first positive late-stage results for an individualised mRNA cancer vaccine.

Earlier research had already generated optimism. In January, Moderna and Merck reported five-year data indicating that the combination reduced the risk of recurrence or death by 49% compared with Keytruda alone.

The latest Phase 3 results provide further support for the possibility that personalised mRNA vaccines could eventually become part of cancer treatment following surgery.

Treatment Could Move Towards Regulatory Review

Moderna said discussions with regulators are already underway. The treatment has received breakthrough therapy designation, potentially accelerating its development and review process.

However, regulatory approval is still required before the vaccine can become available for routine clinical use.

Several questions also remain, including the treatment’s long-term survival benefits, manufacturing requirements, scalability and cost.

What Comes Next for Cancer Vaccines?

The implications of the trial could extend beyond melanoma. Researchers are exploring personalised cancer vaccine approaches for other cancers, including lung, breast and pancreatic tumours.

For patients with high-risk melanoma, preventing the disease from returning after surgery remains a major treatment objective. A personalised vaccine could provide another tool by training the immune system to recognise mutations associated with the patient’s tumour.

For Moderna, the development is also important as the company seeks to expand its mRNA technology beyond COVID-19 vaccines. For Merck, a successful combination could further strengthen Keytruda’s role in cancer treatment.

The full clinical data will be important in determining how significant the breakthrough ultimately proves to be. For now, the positive late-stage results represent a major step forward for personalised cancer treatment and mRNA cancer vaccine research.
